Synopsis

A COMMUTER FLC ABORTED TKOF AFTER NOTING PWR FLUCTUATIONS DURING THE TKOF ROLL. THE BLEEDS HAD BEEN LEFT ON; SO THE CREW THEN COMPLETED THE CHKLIST ITEM AND DEPARTED WITHOUT A NEW RELEASE.

Narrative

LFT TWR GAVE US POS AND HOLD RWY 11. THE BEFORE TKOF CHKLIST WAS REQUESTED AND SUPPOSEDLY COMPLETED. THE FO; WITHOUT PREBRIEFING; CHOSE TO LEAVE THE ENG BLEEDS ON TO PROVIDE COOLING WHILE HOLDING IN POS. DUE TO THE LENGTH OF TIME ON THE RWY; HOLDING; HE FORGOT TO TURN THE BLEEDS OFF. ON THE TKOF ROLL; THE ENG PARAMETERS WERE ERRATIC; AND I CHOSE TO ABORT. WE TAXIED CLR AND COMPLETED THE CHKLIST. THEN WE REQUESTED TAXI TO ANOTHER RWY. I WAS UNDER THE IMPRESSION THAT THE DISPATCH RELEASE WAS STILL GOOD; BUT DUE TO ABORT PROCS IT WAS NOT. THE OTHER DISCREPANCY WAS FAILURE TO COMPLETE THE CHKLIST. BOTH CAPT AND FO ARE DEQUALIFIED AND WILL RECEIVE GND SCHOOL. SUPPLEMENTAL INFO FROM ACN 339692: WE WERE CLRED FOR TKOF AND GIVEN AN ASSIGNED HDG AND A NEW ASSIGNED ALT. I THEN REPEATED THE CLRNC AND SCROLLED IN THE NEW ALT IN THE AFCS. BEING DISTRACTED BY THE NEW ALT; I NEGLECTED TO TURN OFF THE BLEEDS FOR TKOF. THE CAPT ABORTED THE TKOF AT ABOUT 90 KTS. TO MAKE SURE THIS DOESN'T HAPPEN AGAIN; WHEN INITIATING THE BEFORE TKOF CHKLIST; I WILL FULLY COMPLETE IT INSTEAD OF 'HOLDING ON THE BLEEDS.' I'VE ALSO REVIEWED THE COMPANY FLT OPS MANUAL AND HAVE BECOME MORE FAMILIAR WITH THE POLICIES AND PROCS REGARDING ABNORMAL PROCS.
