Synopsis

CAPT OF A BEECH 1900D; AIRLINER; FAILED TO OBTAIN A RE- RELEASE AFTER ABORTING TKOF DUE TO A PRESSURIZATION PROB WARNING. A SUBSEQUENT TKOF WAS MADE WITHOUT CONTACTING DISPATCH AND MAINT FOR A NEW RELEASE AS DIRECTED BY THE COMPANY OPS MANUAL.

Narrative

DURING THE TKOF ROLL; THE L 'ENVIRONMENTAL FAIL' WARNING LIGHT ILLUMINATED. AS I HAD BRIEFED; WE ABORTED THE TKOF. AFTER CLRING THE RWY AND NOTIFYING ATC; WE RAN THE REJECTED TKOF CHKLIST; THEN THE 'ENVIRONMENTAL FAIL' CHKLIST. THIS CHKLIST TOLD US TO RECYCLE THE AFFECTED BLEED AIR CTL. THIS RESULTED IN THE ANNUNCIATOR BEING EXTINGUISHED. THE CHKLIST STATED THAT IF THE LIGHT EXTINGUISHED TO CONTINUE FLT. I MISTAKENLY ASSUMED THIS MEANT THAT NO FURTHER ACTION WAS REQUIRED AND ADVISED TWR THAT WE WERE READY TO DEPART. I DID NOT DISCOVER MY ERROR UNTIL LATER ON THAT DAY WHEN I CHKED OUR FLT OPS MANUAL WITH REGARD TO FILING A PLT IRREGULARITY RPT WITH THE COMPANY. AT THIS TIME I REALIZED THAT I HAD NOT CONTACTED OPS TO AMEND MY DISPATCH RELEASE. I PROCEEDED TO CONTACT THE CHIEF PLT AND HE ADVISED ME TO FILE THIS RPT. I FEEL THAT THE GREATEST FACTOR IN THIS INCIDENT IS MY INEXPERIENCE. I ACTED AS A FO ON THIS ACFT WITH THIS COMPANY FOR 8 MONTHS AND ACQUIRED 427 HRS PRIOR TO MY UPGRADE TO CAPT. AFTER TRAINING; I COMPLETED APPROX 27 HRS OF IOE AND APPROX 30 HRS OF LINE FLYING DURING WHICH I HAD NEVER ACTUALLY ABORTED THE TKOF. I WAS UNDER THE MISUNDERSTANDING THAT IF THE ABORT DID NOT REQUIRE A MAINT ACTION AND WAS STILL WITHIN THE VALID TIME; THERE WAS NO AMENDMENT REQUIRED TO THE RELEASE. I AM NOW AWARE OF EXACTLY WHAT IS REQUIRED AFTER AN ABORT AND/OR AIR RETURN.
