Synopsis

AN AIRLINE B727 HAD MULTIPLE EQUIP PROBS THAT DISTR THE CREW. MAINT CAME ON; DURING WHICH TIME THE PIC RESET THE BRAKES. WHEN READY TO PUSH AGAIN THE GND CREW DID NOT KNOW THE BRAKES WERE RESET. TUG DRIVER ATTEMPTED TO PUSH; BREAKING NOSE GEAR LINKAGE.

Narrative

DURING PUSHBACK; MINOR DAMAGE OCCURRED TO THE NOSE GEAR DOOR LINKAGE DUE TO PARKING BRAKE BEING SET. CONTRIBUTING FACTORS INCLUDED THE FOLLOWING: 1) 12+ HRS INTO DUTY DAY. 2) ARRIVING FLT WAS OVER 1 HR LATE RESULTING IN MINIMAL TURN TIME. 3) MCO ITEMS INCLUDED APU INOP; L PACK INOP; AUTO PRESSURIZATION INOP; AND SEVERAL OTHER MINOR DISCREPANCIES. 4) PREVIOUS FLT EVACED ACFT DUE TO SMOKE IN CABIN CAUSED BY FAULTY APU. 5) DURING PREFLT; I NOTICED REPAIR TAPE PEELING OFF AS WELL AS A MAINT STATION LOGBOOK RELEASE REQUIRED. MAINT WAS NOTIFIED; REPAIRED THE TAPE; SIGNED OFF THE LOGBOOK; AND I ASSUMED THEY COMPLETED THE MEL REQUIRED APU MAINT STATION ITEMS SINCE THEY SIGNED OFF THE LOGBOOK. 6) THERE WERE NUMEROUS INTERRUPTIONS DURING THE BEFORE START CHKLIST. PUSHBACK/START CHKLIST WAS ACCOMPLISHED AT THE GATE. 7) ALL 3 ENGS WERE STARTED AT THE GATE DUE TO APU INOP. 8) AFTER ENG START AND PRIOR TO PUSHBACK; I NOTICED APU LIGHT ILLUMINATED AND MEL ITEMS NOT COMPLIED WITH BY MAINT. 9) CAPT SET BRAKES AND MAINT BOARDED ACFT TO SECURE APU. 10) WE WERE CLRED BY RAMP TWR TO PUSH AFTER AN ACFT CLRED OUR TAIL AND MAINT DEPLANED. 11) CAPT INFORMED RAMP PERSONNEL 'WE'RE CLRED TO PUSH AFTER THE ACFT PASSES BEHIND US.' 12) RAMP GND CREW INITIATED PUSH; BRAKES WERE STILL SET; AND DAMAGE OCCURRED TO NOSE DOOR LINKAGE.
