Synopsis

A B737-200 WAS DISPATCHED WITH THE ACCESSORY DRIVE CASE OIL XFER TUBES MISSING RESULTING IN NO OIL FLOW TO THE #1 BEARING.

Narrative

OCT/XX/97; ACFT XYZ #1 ENG CHANGE. N1 ACCESSORY DRIVE NEEDED REPLACING. I ASSIGNED A MECH TO REPLACE IT. HE DID AND FAILED TO INSTALL THE XFER TUBES. THE ACFT FLEW TO WWW AND OIL IN THE INLET WAS NOTICED THE NEXT DAY. I FLEW TO WWW AND TROUBLESHOT THE PROB; REPLACED THE XFER TUBES; LEAK CHK GOOD. MY COMPANY DECIDED TO CHANGE THE INLET GUIDE VANES AND BEARING IN CASE THE BEARING DIDN'T GET PROPER OIL. THE PROB STARTED WHEN AN ACCESSORY CASE WAS SHIPPED IN DURING PREVIOUS AND THE MECHS AT THAT TIME DISCOVERED IT WAS THE WRONG TYPE OF CASE. THEY ORDERED A NEW ONE AND WE RECEIVED IT ON OUR SHIFT. APPARENTLY PREVIOUS SHIFT PUT 2 OF THE XFER TUBES IN THE FIRST CASE WE RECEIVED AND DIDN'T PUT THEM BACK IN THE OLD CASE WHEN THEY DISCOVERED THAT IT WAS THE WRONG ONE. I ASSIGNED A MECH TO INSTALL THE NEW CASE WE RECEIVED ON OUR SHIFT AND WASN'T AWARE OF HOW MANY XFER TUBES WERE NEEDED AND SWAPPED THE PARTS THAT EXISTED IN THE OLD CASE TO THE NEW. ANOTHER MECH WAS ASSIGNED AT THIS TIME TO HELP WITH INSTALLATION; THEY INSTALLED THE CASE AND WENT HOME WITHOUT SIGNING FOR THEIR WORK. I STAYED AFTER MY SHIFT TO FINISH THE ACFT AND ENDED UP SIGNING FOR THEIR WORK.
