Synopsis

AN MD88 UNDERGOING MAINT WAS FOUND BY AN ACI TO HAVE CIRCUIT BREAKERS RESET AND PLACARDS REMOVED IN CONFLICT WITH THE JOB CARD.

Narrative

ACFT FAILED BOROSCOPE #1 ENG OCT/XX/97; AND ENG CHANGE WAS STARTED AT XA30 AM. THERE WAS AN EXTERNAL PWR MONITOR CHK BEING DONE ON THE ACFT PRIOR TO THE START OF THE ENG CHANGE. AT THIS TIME NO PWR WAS AVAILABLE TO BE ON THE ACFT. THE BREAKERS WERE PULLED AT THIS TIME AND THE CIRCUIT BREAKER PANELS WERE PLACARDED WITH AN MCO (MAINT CARRY OVER) PROC PLACARD. AT XD00 THE ACFT WAS REPOSITIONED AND PWR WAS NOW ON THE ACFT. THE ENG CHANGE WAS IN WORK ALONG WITH OTHER CHK ITEMS. THE EXTERNAL PWR MONITOR CHK WAS COMPLETE AT THIS TIME. AT XC00 ON THE (DATE); THE FAA LCL INSPECTOR ON MD88'S FOUND THE BREAKERS NOT TO BE PULLED OR IDENTED AND STARTED HIS INQUIRIES AT THIS TIME AND AN INVESTIGATION IS NOW UNDER WAY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE BREAKERS WERE PULLED (OPENED) AND PLACARDED PER THE JOB CARD BY THE RPTR ON HIS SHIFT. THE RPTR SAID WHEN THE INSPECTOR CHKED THE CIRCUIT BREAKER PANEL DURING THE NEXT SHIFT IT WAS DISCOVERED THE BREAKERS WERE RESET AND THE PLACARDS REMOVED BUT NO SIGN-OFF WAS MADE ON THE JOB CARD. THE RPTR RELATES BEING CALLED IN WITH AN ACR MGR AND THE ACI FOR A FEW INFORMAL QUESTIONS. THE RPTR SAID AFTER TALKING WITH THE MGR AND INSPECTOR THE RPTR WAS INFORMED OF AN INVESTIGATION AND A LETTER OF REPRIMAND WAS TO BE PLACED IN THE RPTR'S FILE.
