Synopsis

A BEECH 18 INFLT DECLARED AN EMER AND DIVERTED WITH 1 ENG SHUT DOWN DUE TO NO PWR LEVER RESPONSE CAUSED BY A BOLT VIBRATING OUT OF THE FUEL CTL LINKAGE.

Narrative

THE ACFT WAS INFLT WHEN FLC NOTICED NO ENG RESPONSE TO PWR LEVER SETTING. ENG WAS SHUT DOWN AND ACFT LANDED SAFELY. ACFT SHUT DOWN ON RWY (REMAINING ENG) AND WAS TOWED OFF THE RWY. ON INSPECTION; BOLT FOUND MISSING FROM THROTTLE LINKAGE TO FUEL CTL. LAST MAINT PERFORMED WAS ACFT INSPECTION WHERE THE BOLT WAS REMOVED FOR INSPECTION. IT WAS CONCLUDED BOLT WAS NOT SAFETIED PROPERLY AND THUS VIBRATED OUT. ON DISCUSSION BY SHIFT SUPVRS IT WAS DECIDED THAT INSPECTIONS HAD BEEN RUN POORLY BY PASSING WORK FROM ONE SHIFT TO ANOTHER. AS A RESULT SOME WORK DIDN'T GET INSPECTED; THUS PRESENTING A SIGNIFICANT SAFETY HAZARD. ACTIONS TAKEN TO PREVENT THIS FROM HAPPENING AGAIN ARE THE STRESSING OF THE 2 MAN MAINT CONCEPT. EVERYTHING IS LOOKED AT TWICE AND WORK WAS NO LONGER PASSED OFF TO THE NEXT SHIFT FOR COMPLETION; BUT WERE REQUIRED TO BE COMPLETED AND INSPECTED BEFORE THE SHIFT WAS DISMISSED FOR THE DAY. LONGER; MORE COMPLEX TASKS; REQUIRING MORE TIME WERE COMPLETED THE FOLLOWING DAY. ALSO; THE RUSH FROM OPERATIONS WAS LOOKED AT AND LESS EMPHASIS ON SPEED AND MORE ATTENTION TO DETAIL AND QUALITY WERE STRESSED.
