Synopsis

AN L1011-100 WAS DISPATCHED WITH A L WING FUEL QUANTITY TANK PROBE COVER PLATE NOT FASTENED.

Narrative

I PERFORMED ON THE 2L FUEL QUANTITY SYS ON APR/XA/98; AT APPROX XA00 EST. THE TANK WAS DEFUELED AND SUMPED. ALL PROBES IN 2L TANK WERE CHKED FOR INTEGRITY. AS PER MAINT COORD THE FUEL PROBES; NUMBERS 5 THROUGH 9; WERE REPLACED ALONG WITH 2L SIGNAL CONDITIONER; THEN THE TANK QUANTITY INDICATORS WERE RECALIBRATED TO ZERO QUANTITY. ALL MAINT WAS PERFORMED PER MAINT MANUAL. THE ACFT WAS THEN MOVED TO THE ACR TECH OPS CTR FOR A #3 ENG CHANGE. THE ACFT WAS RETURNED TO SVC ON APR/XE/98; APPROX 36 HRS AFTER FUEL TANK MAINT WAS PERFORMED. ON APR/XC/98; ZZZ TWR RPTED THAT ON TKOF SOMETHING WAS STREAMING FROM THE L OUTBOARD WING. VISUAL INSPECTION NOTED FUEL COMING FROM THE TOP OF THE WING. ACFT RETURNED TO ZZZ. ZZZ MAINT FOUND MISSING FASTENERS AT THE #5 FUEL PROBE; ON THE 2L FUEL TANK. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE FUEL QUANTITY WORK WAS ACCOMPLISHED SOLELY BY THE RPTR AND IS SURE ALL THE TANK PROBE COVERS WERE TORQUED TO MAINT MANUAL LIMITS. THE RPTR SAID THE MECH WHO REPAIRED THE ACFT RPTED 4 FASTENERS WERE MISSING AND ONE WAS INTACT AND TIGHT. THE RPTR STATED THE FAA HAS MADE CONTACT.
