A B747-100 ON APCH AT 11000 FT DECLARED AN EMER DUE TO A SMOKING LNDG LIGHT SWITCH PANEL AND NUMEROUS OTHER SYS AND COMPONENT FAILURES PREVIOUSLY RPTED AND NOT CORRECTED BY MAINT.

Narrative

UP AT XA00 FOR AN XB00 CREW BUS PICKUP. 1 HR RIDE TO ARPT. CHKED IN AT OPS AND ARRIVED AT ACFT AT XC15. BLOCKED OUT AT XE06. DURING CTL CHK; WHILE TAXIING; DDPI SHOWED NO MOVEMENT OF L OUTBOARD AILERON. CHANGED FLAP POS AND TRIED SEVERAL MORE TIMES. VISUAL CHK CONFIRMED NO MOVEMENT OF L OUTBOARD AILERON. RETURNED TO GATE AT XE40. MECHS REMOVED AND REPLACED L OUTBOARD AILERON LOCKOUT MECHANISM. THIS HAD BEEN WRITTEN UP PREVIOUSLY AS ERRATIC INDICATION. I DO NOT BELIEVE THAT A VISUAL CHK WAS MADE. ACFT MAY HAVE BEEN FLOWN WITH AILERON LOCKED OUT. BLOCKED OUT AGAIN AT XI38 FOR 8 HR TRIP TO DUSSELDORF. FLT NORMAL UNTIL PASSING 15000 FT MSL ON DSCNT. FE STATES #4 HYD SYS LOW FLUID LEVEL. SUGGESTS LOWERING GEAR EARLY ON APCH. ALL AGREE. PASSING 10000 FT LNDG LIGHTS TURNED ON. 2 SECONDS LATER; HVY; WHITE; ACRID SMOKE COMING FROM OVERHEAD PANEL. ELECTRICAL SMOKE/FIRE CHKLIST PERFORMED. LNDG LIGHTS TURNED OFF IMMEDIATELY. SMOKE QUITS. IDENT OF SOURCE OF SMOKE NOTED. #3 N1 GAUGE FAILS IN DSCNT. HIGH RPM AND SPD BRAKES USED TO MAINTAIN ANTI-ICING AND DSCNT PROFILE TO ILS. COPLT'S ELECTRIC ALTIMETER FAILS AT 5680 FT. NOTED AT 3000 FT FAF LEVELOFF. SUCCESSFUL APCH AND LNDG MADE TO MINIMUM LENGTH WET RWY. CLRED ACTIVE AND TAXIED TO STAND. FE MAKES 'STATES' PWR XFER TO APU. ON STAND SHUTDOWN OF #4 ENG DROPS ALL LIGHTS IN ACFT. #1 ENG LEFT RUNNING. APU INDICATIONS ZERO. APU AUTO SHUTDOWN DURING TAXI-IN APPARENT. GND PWR CONNECTED. SHUTDOWN COMPLETE. CREW KEPT AT ACFT FOR NOISE. NO COPLT ALTIMETER AVAILABLE. DUTY DAY AFTER 1 HR LIMOUSINE TO HOTEL WAS 19 HRS. ALL THESE FAILURES HAD MAINT WRITE-UPS PREVIOUSLY. MECHS WRITING OFF/DEFERRING WITHOUT MAINT. CRM DOES WORK!
