A DC10-10 BEING PUSHED OFF A GATE FOR TRIP DISPATCH HAD THE #4 FRONT WHEEL HUB CAP FALL OFF DUE TO IMPROPER INSTALLATION.
ON DEC/XA/98 AT XA30; I WAS ASSIGNED TO FINISH A #2 SVC STARTED BY THE TAXI CREW. THE TAXI CREW ALSO INITIATED WRITE-UP ON #4 FRONT BRAKE FOR BEING WORN; WHICH I WAS ASSIGNED AS WELL. THE ACFT WAS SCHEDULED FOR #2 ENG BORESCOPE WHICH WAS ASSIGNED TO 2 OTHER MECHS. AT THE TIME ACFT ARRIVED TO HANGAR (AT XA30) IT WAS STRESSED TO US BY THE MGR THAT THE ACFT HAD TO BE PUSHED OUT OF HANGAR AT XE00 FOR SCHEDULED TRIP. I HAD TO WORK THROUGH LUNCH TO ACCOMPLISH ALL CHKS AND BRAKE CHANGE BEFORE XE00. I STARTED THE BRAKE CHANGE BY MYSELF. AFTER PERFORMING FINAL TORQUE ON WHEEL NUT AND INSTALLATION OF SAFETY BOLT; I INSTALLED THE HUB CAP AND CLAMP ON WHEEL WHILE THE OTHER MECH PUT AWAY OUR SUPPORT EQUIP. AT XD45; WHILE PERFORMING A FINAL WALKAROUND AND CLOSING LNDG GEAR DOORS; I DISCOVERED A HYD LEAK FROM HORIZ STABILIZER COMPARTMENT. AFTER TROUBLESHOOTING THE LEAK DOWN TO A LOWER HORIZ STABILIZER TRIM VALVE; THE MGR TOLD US TO RIDE THE ACFT TO GATE AND FIX IT THERE. WE TURNED THE ACFT OVER TO GATE MECH AT XF30. ON DEC/XB/98 AROUND XA00; I WAS INFORMED THAT THE #4 FRONT HUB CAP HAD FALLEN OFF THE WHEEL WHILE BEING PUSHED BACK FROM THE GATE FOR A LIVE TRIP. THE ACFT WAS BROUGHT BACK TO CHOCKS AND A NEW CLAMP WAS INSTALLED; DELAYING THE TRIP FOR ANOTHER HR. THE OLD CLAMP WAS FOUND OUTSIDE OF HANGAR SOMETIME DEC/XB/98 IN THE EVENING. I WAS SHOWN THE CLAMP WHICH WAS SEVERELY DAMAGED DUE TO ACFT ROLLING OVER IT. I NOTICED THE NUT BEING BACKED OFF ALL THE WAY TO THE END OF THREADS. I REMEMBER; I DIDN'T HAVE TO UNSCREW THE NUT THAT FAR BACK TO REMOVE THE CLAMP OR TO INSTALL IT.
