Synopsis

A B757-200 FLT DOES NOT FOLLOW THE MLF 2 DEP PROC OUT OF SLC; UT.

Narrative

DEPARTING SLC; WE WERE ASSIGNED A HDG DURING CLB PHASE TO JOIN THE MLF.2 DEP. WE THOUGHT THE FMS WAS PROGRAMMED CORRECTLY; BUT APPARENTLY NOT. DURING A BUSY TIME SUCH AS CLBOUT; WE FLEW THROUGH OUR COURSE. I'M NOT SURE EXACTLY HOW MANY MI; BUT I DO KNOW WE DID NOT FLY THROUGH ANY RESTR AREAS; OR GET IN THE WAY OF ANY ACFT. TCASII WAS CLR OF ACFT IN 40 NM RANGE. IT TOOK ONLY A MINOR CORRECTION; ABOUT A 30 DEG R TURN TO INTERCEPT. INTERCEPTION OF RTE WAS VERY QUICK. THE PROB; I BELIEVE; WAS A RESULT OF WORKLOAD. DURING THE PREPARATION PRIOR TO PUSHBACK; FUELING WAS LATE; PAPERWORK ALL AROUND COMING AT US; AND OUR FLT PLAN IS ALWAYS ENTERED AND DOUBLECHKED. HOWEVER; A PDC A DEP IS GIVEN AND IT ALSO LISTED FLT PLAN RTE. WITH EVERYTHING HAPPENING AT ONCE AND ABOUT 3-4 FMS PROCS STILL NEEDED TO BE COMPLETED. AFTER FUELING I BELIEVE THE DEP WAS ENTERED BUT NOT CORRECTLY. MAYBE A 'TRANSITION' WAS NOT PICKED. BOTTOM LINE IS: HIGH WORKLOAD WITH FMS AT CRUCIAL TIMES -- SOMETIMES LEADS TO MISTAKES. THIS IS WHAT I BELIEVE -- NOT KNOWING THE PROB FOR SURE.
