Synopsis

A DORNIER 328 ON TAXI OUT DECLARED AN EMER AND EVACED THE ACFT DUE TO SMOKE AND FUMES CAUSED BY AN AIR CYCLE MACHINE BEARING FAILURE.

Narrative

NORMAL TAXI WITH BLEEDS ON (AFTER PROPS OUT OF FEATHER FOR 1 MIN). TAXI FROM HORSESHOE TO RWY 25 VIA TXWYS BN AND G. TAXI CHKLIST COMPLETED ON TXWY BN WBOUND. TAXIING S ON TXWY G; CAPT CALLED FOR 'BEFORE TKOF CHK TO THE LINE' AND THE CHKLIST WAS COMPLETED. A BLUE CAUTION ALERTING SYS MESSAGE 'L PACK INOP' APPEARED. CAPT SELECTED ENVIRONMENT CTL SYS PAGE WHILE FO SELECTED THE L ENVIRONMENT CTL SYS FROM 'MANUAL' TO 'AUTO' MODE. CAPT SUGGESTED RESETTING THE L PACK TO ATTEMPT TO CLR THE BLUE CAUTION ALERTING SYS MESSAGE. THE FO TURNED THE L PACK OFF; AND XBLEED ON (2 BLEEDS FEEDING R PACK). ON THE ENVIRONMENT CTL SYS PAGE; AND THEN ON THE CAUTION ALERTING SYS FIELD; AMBER 'L PACK FAIL/OVERHEAT' MESSAGES APPEARED. THE FLT ATTENDANT THEN CALLED ON THE INTERPHONE AND RPTED SMOKE IN THE CABIN. NO SMOKE WAS DETECTED IN THE COCKPIT AT THIS TIME. CAPT ASKED IF EVAC WAS NEEDED. THE CAPT STOPPED THE ACFT AND SET THE PARKING BRAKE. THE COCKPIT DOOR WAS OPENED FOR THE CREW TO ASSESS THE SIT. THE CAPT AND FO VIEWED A WHITE HAZE IN THE PAX CABIN. FO SUGGESTED THAT THE HAZE MIGHT BE CONDENSATION VAPOR FROM THE AIR CONDITIONING. AFTER A MOMENT OF EVALUATION; THE CAPT AND FO BEGAN TO SMELL THE HAZE AND DETERMINED IT WAS NOT CONDENSATION VAPOR. THE CAPT ADVISED THE FLT ATTENDANT TO PLAN AN EVAC OUT THE MAIN CABIN DOOR. THE FO ADVISED ATC OF SMOKE IN THE CABIN AND EVAC PLANE. CAPT EXECUTED THE 'EMER EVAC' MEMORY ITEMS. ONCE THE L ENG AND PROP WAS SECURE; FLT ATTENDANT OPENED MAIN CABIN DOOR AND DIRECTED THE PAX OFF THE ACFT; WHILE THE CAPT CALLED 'EVAC L' ON THE PA. THE FO COMPLETED THE 'EMER EVAC' CHKLIST. CAPT DIRECTED FO TO DEPLANE AND TO ACCOUNT FOR THE PAX. CAPT EXITED LAST WITH FLASHLIGHT AFTER TURNING THE BATTERIES OFF PER CHKLIST.
