Synopsis

A DC10-10; IMMEDIATELY AFTER TKOF; DECLARED AN EMER AND RETURNED TO THE FIELD DUE TO A FIRE WARNING IN #1 ENG CAUSED BY A LOOSE PNEUMATIC DUCT CLAMP.

Narrative

ON AUG/XA/99 I WAS INSTRUCTED BY MY LEAD MECH TO REPLACE THE REVERSER DRIVE MOTOR ON #1 ENG ACFT XYZ. UPON TURNOVER AT XA00; I HAD ONLY REMOVED THE REVERSER DRIVE MOTOR AND OVERPRESSURE SHUTOFF VALVE ATTACHED TO THE DRIVE MOTOR; THEN INSTALLED NEW DRIVE MOTOR WITH THE 4 MOUNT BOLTS. NO DUCTING WAS REMOVED OR LOOSENED OTHER THAN THE 2 FT PIECE DIRECTLY BELOW THE DRIVE MOTOR THAT ATTACHES BTWN THE REGULATOR VALVE AND DRIVE MOTOR. AT THIS TIME; THOROUGH EXPLANATION OF WHAT HAD BEEN DONE WAS GIVEN TO THE NEXT SHIFT CREW ASSIGNED TO CONTINUE THE WORK. THE NEXT DAY; I WAS INFORMED THAT THE ACFT HAD AN INFLT SHUTDOWN BECAUSE OF A FIRE WARNING IN THE FLT COMPARTMENT. AFTER EXAMINATION BY ANOTHER MECH ON THE GATE; I WAS INFORMED THAT THE FAILURE WAS ORIGINATED BY A DUCT CLAMP ON THE BOTTOM OF THE ENG UPSTREAM OF THE REGULATOR VALVE. THIS IS ABOUT 4 FT FROM WHERE I WAS WORKING AND NEVER HAD THE NEED TO LOOSEN ANYTHING ON THE BOTTOM OF THE ENG TO REMOVE THE DRIVE MOTOR. ANOTHER CREW HAS REPLACED THE DAMAGED REVERSERS ALONG WITH OVERPRESSURE SHUTOFF VALVE AND DAMAGED WIRING HARNESS ON AUG/XA/99. THIS ACFT HAS HAD CHRONIC HISTORY PROBS ON #1 ENG REVERSERS.
