Synopsis

AN SF340 CREW; TAXIING AT ALB; BECAME DISTR BY A CAUTION LIGHT CAUSING THE CAPT TO CROSS THE HOLD SHORT LINE OF RWY 28.

Narrative

AFTER AN UNEVENTFUL FLT; WE LANDED ON RWY 10 AT ALB AND EXITED TO THE L ON TXWY D AND THEN ANOTHER L ON TXWY A. AS I WAS APCHING THE INTXN OF TXWY A AND RWY 28; THE CTLR YELLED 'HOLD SHORT' SEVERAL TIMES. HE THEN ATTEMPTED TO GET THE ACFT THAT WAS DEPARTING RWY 28 TO CANCEL TKOF CLRNC. I APPLIED EMER BRAKING AND CAME TO A STOP OVER THE HOLD SHORT LINE; BUT SHORT OF THE RWY. NO PART OF MY ACFT ENTERED THE PHYSICAL RWY. I HAVE BEEN PROFOUNDLY SHAKEN BY THIS INCIDENT! I AM FURTHER DISTURBED BY MY RECOLLECTION OF SOME OF THE EVENTS THAT TOOK PLACE IMMEDIATELY PRIOR TO THE INCURSION. EVERY TIME I HAVE LANDED ON RWY 10 I HAVE TAKEN THE SAME EXIT AND; WITHOUT FAIL; HAVE BEEN TOLD TO 'TAXI TO THE RAMP.' I DO NOT RECALL HEARING THIS INSTRUCTION; NOR DO I RECALL EVER HEARING A 'HOLD SHORT RWY 28' INSTRUCTION. I AM TOLD MY FO READ THE HOLD SHORT INSTRUCTION BACK. MY FO WAS VERY NEW AND JUST OFF IOE. ON TOUCHDOWN HE INFORMED ME THAT THE ANTISKID SYS HAD FAILED. THE ANTISKID MASTER CAUTION LIGHT EXTINGUISHED AND I INFORMED HIM THAT ON A PARTICULARLY SMOOTH LNDG; YOU WOULD GET THAT LIGHT. HE CONTINUED TO ASK QUESTIONS ABOUT THIS SUBJECT UNTIL WE WERE CLR OF THE RWY. I TOLD HIM WE WOULD DISCUSS THIS MATTER FURTHER WHEN WE ARRIVED AT THE RAMP. SINCE WE WERE AFT CTR OF GRAVITY; I TOLD MY FO TO 'GUARD' COM #1 AND I WENT TO COM #2 TO ASK FOR A TAIL STAND. I BELIEVE WHILE I WAS ON COM #2 IS WHEN MY FO MAY HAVE RECEIVED THE HOLD SHORT INSTRUCTION. IF THIS HAPPENED; I WAS NOT UPDATED AS TO ITS RECEIPT. SINCE OUR COMPANY IS ALLOWED TO SCHEDULE REDUCED REST; AND SINCE REDUCED REST DOES NOT PROVIDE ADEQUATE REST; I MAY HAVE BEEN HURRYING. CHANGES I HAVE MADE TO COCKPIT PROC ARE: 1) WHILE THE ACFT IS MOVING; ALL COMS WILL BE ON 1 RADIO. THIS WILL ENABLE ME TO MONITOR ALL INSTRUCTIONS. 2) ALL COMPANY BUSINESS WILL BE CONDUCTED ON THE RAMP. NO BUSINESS IS SO IMPORTANT TO REQUIRE SEPARATE CALLS. 3) MAKE A CONSCIOUS EFFORT TO BE SLOW AND DELIBERATE. 4) LIMIT EVEN ACFT SPECIFIC CONVERSATION UNTIL ACFT IS STOPPED.
