Synopsis

AN ACR FLEW 2 LEGS WITH IMPROPER MAINT DOCUMENTATION. IT WOULD NOT HAVE HAPPENED EXCEPT FOR A CREW BEHAVIOR PROB.

Narrative

DURING PREFLT AT IND; THE CAPT NOTICED THAT THE LOGBOOK CONTAINED A WRITE-UP FROM THE PREVIOUS CREW AND CALLED MAINT OUT TO LOOK AT IT. MAINT DEFERRED THE ITEM AND ENTERED THE MEL. THE CAPT ASKED THE FE TO DOUBLECHK THE ITEM IN THE MEL BOOK. HE (THE FE) STATED EVERYTHING WAS GOOD. WE DEPARTED IND TO MMUN THEN ON TO TNCA. AT TNCA WE NOTICED OUR NEW PAPERWORK'S MEL ITEM WAS INCORRECT (341800 DME VERSUS 342200 #1 ADF). THE CAPT ASKED THE FE TO DOUBLECHK AGAIN THE MEL BOOK FOR THE CORRECT NUMBER AND WHY IT WASN'T CAUGHT THE FIRST TIME. THE FE ADMITTED HE DID NOT CHK THE BOOK THE FIRST TIME BACK IN IND. WHEN ASKED WHY; HE LOUDLY (AND WRONGLY) PROTESTED THAT HE DIDN'T NEED TO SINCE MAINT CTL HAD THE LOGBOOK AT THE TIME (LOOKING AT THE WRITE-UP) AND MEL'S WERE THEIR JOB. WE FLEW 2 LEGS WITH A WRONG MEL NUMBER ON PAPERWORK. AT TNCA; CAPT CALLED COMPANY AND GOT NEW TIME/INITIALS TO ADD TO NEW PAPERWORK AND OLD TO REPLACE MEL NUMBER. CONTRIBUTING FACTORS: MAINT CTL INPUTTING WRONG MEL ITEM. FE SAYING HE CHKED MEL BOOK AND IT WAS GOOD WHEN HE HADN'T. CAPT AND FO NOT ASKING TO SEE MEL BOOK ALSO TO DOUBLECHK. FACTORS TO HUMAN PERFORMANCE: FE IS 20 YR EX-CAPT WHO ALIENATES EVERYONE ELSE. HE HAS NO CRM SKILLS. CAPT TOLD HIM THAT HE SHOULD HAVE LOOKED IT UP AS ASKED. FE FIRMLY STATED HE DIDN'T NEED TO.
