AN EMBRAER 120 WAS RPTED TO HAVE CHRONIC HISTORY OF SMOKE IN THE CABIN AND R DUCT OVERHEAT RPTS WITH CREW CONCERN OF MAINT INDIFFERENCE.

Narrative

AT GATE; BTWN SCHEDULED FLTS; APPROX 3 MINS AFTER MAIN ENGS SHUT DOWN; WITH APU ON DURING POSTFLT; AFTER ALL PAX HAD DEPLANED; WITH AIR CONDITIONING PACKS OFF (DUSK); SMELLED SMOKE; DETECTED WARMING (RECIRCULATION AND GASPER FAN ON); WITNESSED LAVATORY SMOKE DETECTION ACTIVATION CONTINUOUSLY. SET BOTH PACKS LOW AND CONFIRMED COOLING. SMOKE CONDITION CONTINUED. SHUT OFF BOTH PACKS AND ALL RECIRCULATION AND GASPER FANS; LEFT COCKPIT TO INSPECT CABIN WITH FLT ATTENDANT; AND WHILE IN CABIN THE R DUCT LEAK WARNING OCCURRED; NO MORE THAN 5 MINS AFTER PARKING. RETURNED TO COCKPIT; CHKED APU PANEL (NO DUCT LEAK WARNING); AND SHUT DOWN APU; LEAVING BATTERY ON; THEN FINALLY GND PWR TO MONITOR FAULT; WHICH REMAINED ANNUNCIATED; APPROX 30 MINS. MAINT FINALLY ARRIVED ABOUT XP00; BUT FAULT WOULD NOT DUPLICATE WITH APU; BOTH PACKS; AND ALL FANS OPERATING. ACR X MAINT CTL XXX DENIED FERRY FLT AUTH APPROX XP30; PENDING ARR OF ACR X MECH AT XQ30 ON FLT FROM YYY. AT XQ00 CALLED PLT SCHEDULING FOR RELEASE FROM DUTY WHICH STARTED AT XB30 (ACTUALLY XA30 PNS); RELEASE 'GRANTED' AT XQ30 FOR UNSCHEDULED OVERNIGHT. AT XR00 HOTEL CHK-IN; HAD MESSAGE TO CALL YYY MAINT. WHEN CALLED BACK; I DECLINED THE REQUEST TO FERRY THE PROB ACFT ZZZ TO YYY. BY XR15 HAD RECEIVED CALL FROM CREW SCHEDULING ADVISING XA30 RPT. SHORTLY AFTER XA00 ON FEB/THU/01 DURING PREFLT; FOUND OIL LEAK #1 ENG AND AFTER INSPECTIONS AND ENG RUNS; AS THE SUN PEAKED OVER THE TERMINAL AND SHINED IN THE OPEN COWLING; AN OIL DRAIN LINE FROM THE STARTER GENERATOR 'GARLOCK' SEAL VICINITY WAS SEEN TO BE DAMAGED AND LEAKING. THE REPAIR WAS NOT COMPLETED AND WE DEPARTED ON FERRY FLT AT XC30 FROM ZZZ ARRIVING YYY AT XD46. OVERNIGHT; THE ACR X MECH HAD AUTH FERRY FLT ON FEB/WED/01 FROM ZZZ TO YYY FOR THE FEB/WED/01 DUCT LEAK WARNING MAINT ITEM. SINCE THIS FERRY; I DECLINED TO CONDUCT AS IT WOULD INVOLVE FLYING AFTER BEING ON DUTY MORE THAN 16 HRS; THE ACR X MECH HAD TIME EARLY MORNING ON FEB/THU/01 TO PERFORM A SECOND TROUBLESHOOTING INSPECTION; WHICH WAS DOCUMENTED AS SUCH WITH RESULTS 'COULD NOT DUPLICATE.' PRIOR TO THE ABOVE OIL LEAK DISCREPANCY AND REPAIR; THIS ACFT (BRASILIA) WAS CLRED OF ANY MAINT DISCREPANCY ITEMS RELATED TO 'R DUCT LEAK WARNING;' AND WAS CERTIFIED MECHANICALLY REVENUE WORTHY. UPON PARKING THE ACFT AT YYY HANGAR; BEFORE THE FLC COULD EXIT THE COCKPIT DUE TO A MAINT SUPVR BOARDING AND IN THE DOORWAY; THE SMOKE SMELL RETURNED; SMOKE ALARM SOUNDED; THEN THE R DUCT LEAK WARNING DUPLICATED ITSELF IDENTICALLY TO THE OCCURRENCE THE PREVIOUS EVENING; ALL WITH BOTH PACKS OFF AND ALL RECIRCULATION AND GASPER FANS OFF. MY MAIN 2 CONCERNS ARE: 1) THE PLT PUSHING; EXPECTING AND ASKING ME TO FLY FAULTY EQUIP AFTER MORE THAN 16 HRS DUTY; AND 2) THE SEEMING RELUCTANCE OF ACR X MAINT CTL XXX TO PROVIDE PROMPT NECESSARY MAINT. SHIP XXXX HAS HISTORY OF SMOKE ALARM ACTIVATION AND APU BLEED FOULING AIR CONDITIONING SYS.
