Synopsis

A BE40 CREW EXPERIENCED AN ENG FAILURE ON DEPARTURE.

Narrative

AFTER TKOF; AT 500 AGL WE EXPERIENCED A LEFT HAND ENG FIRE WARNING LIGHT ANNUNCIATION ALONG WITH THE AURAL FIRE WARNING SYS BELL. THE ENG FIRE MEMORY ITEMS WERE COMPLETED IN CONJUNCTION WITH DECLARING AN EMER AND APPRISING THE TWR OF OUR ANOMALY. ONCE THE LEFT ENG THRUST LEVER WAS RESTARTED THE LEFT FIRE WARNING LIGHT EXTINGUISHED. DUE TO THE FACT THAT THE LEFT HAND ITT WAS SHOWING FLUCTUATIONS OF 5O DEG WE ACCOMPLISHED A PRECAUTIONARY LEFT HAND ENG SHUT-DOWN; AND LANDED WITH NO FURTHER PROBS. AFTER LANDING WE CLRED THE RWY AND FO ESCORTED THE SIX PASSENGERS FROM THE ACFT AS A PRECAUTION. VMC CONDITIONS PREVAILED AT THE ARPT WITH WINDS NORTHERLY AT 11 KNOTS AND 20 MILES VISIBILITY. UPON CALLING THE TWR; I ASKED THE CTLR IF ANY STATEMENT WAS REQUIRED BY THE CREW AND WE WERE TOLD NO PAPER WORK WOULD BE REQUIRED. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THAT A MAINT CHK REVEALED THAT THE P-3 TUBE; I.E. BLEED AIR DUCT; HAD FAILED CAUSING THE FIRE WARNING. MAINT INDICATED THAT TWO SEPARATE COUPLINGS HAD FAILED DUE TO IMPROPER SEATING OF METAL GASKETS. THERE WAS NO SIGN THAT A FIRE HAD OCCURRED. PREVIOUS MAINT HAD BEEN PERFORMED BY COMPANY.
