Synopsis

A B737-300 ON TAXI AFTER PUSHBACK THE FIRST CAB ATTENDANT ALERTED THE CREW OF THE CABIN FILLING WITH SMOKE. THE ACFT WAS EVACUATED.

Narrative

NORMAL PUSHBACK WITH APU RUNNING; SUPPLYING RIGHT PACK. AFTER CLRNC FROM PUSHBACK CREW; STARTED #2 (RT) ENG USING APU PNEUMATICS. ENG START NORMAL ALL PARAMETERS; NOTED SOME SMOKE OUTSIDE ACFT ON ENG START; NOT ABNORMAL. PUSHBACK CREW REQUESTED AND CLRED TO DISCONNECT. ALL CHECKLIST ITEMS ACCOMPLISHED FOR SINGLE ENG TAXI; RIGHT PACK SUPPLIED BY #2 ENG PNEUMATICS; LEFT PACK SUPPLIED BY APU PNEUMATICS. RAMP CTL GAVE CLRNC TO CONTACT GND FOR EXIT TXWY. FO CONTACTED GND; WE WERE INSTRUCTED TO TAXI OUT TO SPOT X HOLD SHORT TXWY. AFTER CLRING LEFT AND RIGHT; I ADVANCED RIGHT THRUST LEVER TO APPROX 80 PERCENT N2 FOR BREAKAWAY. AFTER ABOUT 10 SECS; HEARD THREE CHIMES ON CAB INTERPHONE; AND BEFORE I COULD SELECT CAB INTERPHONE ON AUDIO SELECT PANEL; THREE MORE CHIMES. ANSWERED AND HARD FA SAY THERE WAS SMOKE IN THE CAB AND IT WAS GETTING THICK AND BEGINNING TO STING THE EYES. SHE THEN SAID THAT THEY NEEDED TO GET OUT. SIMULTANEOUSLY WITH ALL OF THIS THE FO SECURED BOTH PACKS AND BLEEDS AND SHUTDOWN #2 VHF RADIO (RAMP CTL FREQ) AND ANNOUNCED THAT WE WERE EVACUATING THE ACFT. I THEN MADE AN EVACUATION PA. THE FO GOT UP OUT OF HIS SEAT AND REMOVED THE SECURITY BAR FROM THE COCKPIT DOOR AND OPENED THE DOOR. I INITIATED EVACUATION CHECKLIST ITEMS; AND EXITED COCKPIT. I OBSERVED NO RESIDUAL SMOKE; THOUGH THERE WAS SOME REMAINING ODOR. EVACUATION OF PAXS WAS PROCEEDING OUT THE 1L AND 2L DOORS. ALL SLIDES WERE DEPLOYED (1L; 2L; 1R; 2R) ALTHOUGH THE 1R DOOR WAS ONLY PARTIALLY OPEN AND THE SLIDE HAD HUNG UP. NO ONE WAS EXITING THE OVER WING EXITS; AND HOSE DOORS REMAINED CLOSED. I DIRECTED THE FO TO EXIT 1L AND ASSIST THE PAXS AT THE BOTTOM OF THE SLIDE. ACR RAMP; PAX ASSISTANCE AND MAINT PERSONNEL WERE ON SCENE THROUGHOUT AND DIRECTING PAXS BACK TO THE TERMINAL THROUGH THE NEAREST JETWAY. I REMAINED AT THE 1L DOORWAY WITH THE FA AND TOOK CARRY ON LUGGAGE FROM EVACUATION PAXS; STOWING THE LUGGAGE IN THE FORWARD GALLEY. AFTER THE LAST PAX HAD EXITED FORWARD; I MADE A SWEEP THROUGH THE CAB FOR STRAGGLERS; (ACCOMPANIED BY UNIFORMED DEADHEAD ACR PLT DIRECTED THE AFT FLT ATTENDANTS FA #2 AND FA #3 TO EVACUATE. I RTNED FORWARD; AND EXITED THE ACFT LAST. I OBSERVED FIRE AND RESCUE VEHICLES ARRIVING ON SCENE AT THIS TIME. ACR MAINT PERSONNEL ASSUMED CTLR OF THE ACFT AND TOWED IT BACK TO THE GATE. CALLBACK CONVERSATION WITH THE RPTR REVEALED THE FOLLOWING INFO: THE RPTR STATED THE PRELIMINARY RPT FROM MAINT ON THE CAUSE OF THE EYE STINGING SMOKE WAS OIL FROM #2 ENG IN THE PNEUMATIC BLEED AIR SUPPLYING THE RIGHT PACK. THE RPTR SAID WHAT FAILED ON THE ENG TO ALLOW OIL INTO THE BLEED AIR WAS NOT RPTED. THE RPTED STATED ALL EMER EQUIP OPERATED EXCEPT #1 RIGHT SVC DOOR SLIDE FAILED TO DEPLOY DUE TO THE DOOR NOT OPENED FULLY. THE RPTR SAID NO INJURIES WERE INCURRED.
