Synopsis

B777 CREW LANDED AT DFW WITHOUT A CLRNC.

Narrative

ON APCH TO RWY 13R DFW. CAPT FLYING. FO DOING RADIO. RELIEF PLT MONITORING. CLRED FOR APCH; NOT SURE IF TOLD TO CONTACT TWR AT MARKER. ASSUMED FO MADE FREQ CHANGE AND DID NOT NOTICE IF WE WERE CLRED TO LAND. AFTER LNDG; I ASKED FO IF HE SWITCHED TO GND. HE THEN REALIZED WE WERE STILL ON APCH FREQ. WE THEN GOT ON THE CORRECT FREQ AND OPS NORMAL AFTER THAT. THERE WERE NO RWY INCURSIONS. I DIDN'T KNOW WE WERE TAXIING ON WRONG FREQ FOR A SHORT TIME. MOST FLEETS HAVE TECHNIQUES TO FLIP A SWITCH OR OTHER METHODS OF REMINDING CREW THEY HAVE BEEN CLRED TO LAND. I DON'T KNOW OF ONE ON THE B777; BUT I THINK THERE SHOULD BE ONE (I THOUGHT THIS BEFORE THE EVENTS OF TODAY). IN THE PAST; I HAVE THOUGHT IT A GOOD IDEA TO HAVE A CHKLIST ITEM FOR CLRED TO LAND. IF WE HAD SUCH A TECHNIQUE OR PROC; I AM SURE WE WOULD HAVE CAUGHT THIS ERROR BEFORE WE LANDED. SUPPLEMENTAL INFO FROM ACN 540689: CLRED ILS RWY 13R TO CONTACT TWR AT FIX. SET IN CORRECT FREQ BUT DID NOT SWITCH TO TWR AT FIX. ABOUT THAT TIME WE WERE TALKING ABOUT WIND CORRECTION SPD. I JUST FORGOT TO MAKE THE SWITCH. NORMAL NO CONFLICT LNDG. ON THE GND WE REALIZED WE HAD NOT SWITCHED OVER.
