Synopsis

B757-200 CREW DID NOT LOWER THE FLAPS FOR TKOF.

Narrative

ON PUSHBACK AT BDL; I DECIDED TO LEAVE THE FLAPS UP FOR TAXI DUE TO SNOW FALLING AND THE TXWYS BEING COVERED BY 2-3 INCHES OF SNOW. I INTENDED TO SET THE FLAPS AS WE GOT CLOSER TO OUR DEP RWY. TAXIING WAS DIFFICULT DUE TO ALL TXWY MARKINGS BEING OBSCURED. AS WE APCHED THE RWY; TWR ASKED IF WE WOULD BE READY AND CALLED OUR ATTN TO THE SNOW REMOVAL EQUIP OFF OUR R WING. WE WERE THEN CLRED TO POS AND HOLD. WE RECEIVED TKOF CLRNC AND AS I ADVANCED THE PWR LEVERS; THE TKOF WARNING HORN SOUNDED. WE ABORTED AND CLRED AT THE FIRST INTXN. MAX SPD WAS 20-30 KTS. PROBS: 1) WE NORMALLY SET THE FLAPS AS WE BEGIN TO TAXI. THIS NORMAL ROUTINE WAS BROKEN. I FEEL THIS IS THE MAJOR FACTOR. 2) TAXIING CONSUMED MORE OF OUR ATTN THEN NORMAL DUE TO ALL TXWY MARKINGS BEING OBSCURED AND REDUCED VISIBILITY. 3) IT WAS THE LAST LEG AND THE LAST DAY OF A 5 NIGHT TRIP (BACK SIDE OF THE CLOCK). WE ARE NEVER AS WELL RESTED AT THE END OF A TRIP AS WE ARE AT THE BEGINNING OF ONE. I FEEL THESE 3 FACTORS COMBINED ARE WHAT LED TO THIS EVENT. MY PREVENTION PLAN IS TO USE A RED STREAMER TO REMIND ME ANY TIME A CTL OR SWITCH IS NOT IN ITS NORMAL POS. THIS WILL SERVE AS A REMINDER THE NEXT TIME I AM IN A SIMILAR SIT.
