Synopsis

An LR25 flight crew flies an inexact ILS approach to runway 20 at IDA, ID.

Narrative

Rewritten in plain English from NASA’s original all-caps telegraphic report — facts, figures, and terminology preserved.

First officer flying ILS into IDA runway 20. Controller informed crew to report outer marker. First officer was getting low on glide slope; so captain informed the first officer to take corrective action.

Approximately 1 mile outside the outer marker, captain started completing the before landing checklist. First officer was still low on the glide slope. Captain informed co-pilot to immediately start climbing to recapture glide slope. At this point, aircraft had passed through the outer marker.

Captain finished completing before landing checklist. At this point, circuit breaker for altitude indicator and HSI popped. Captain momentarily lost instruments until circuit breaker was reset. Aircraft broke out of IMC approximately 400 feet from the ground and 2 miles from runway. Clearance to land was issued by tower and read back by captain.

After a normal landing was made, aircraft exited the runway. Controller told crew a deviation had occurred because of a failure to report the outer marker. No enforcement action was taken by controller. Crew was distracted by position of aircraft on ILS glide slope and momentary loss of flight instruments.

FO FLYING ILS INTO IDA RWY 20. CTLR INFORMED CREW TO RPT OM. FO WAS GETTING LOW ON GS; SO CAPT INFORMED THE FO TO TAKE CORRECTIVE ACTION. APPROX 1 MI OUTSIDE THE OM; CAPT STARTED COMPLETING THE BEFORE LNDG CHKLIST. FO WAS STILL LOW ON THE GS. CAPT INFORMED COPLT TO IMMEDIATELY START CLBING TO RECAPTURE GS. AT THIS POINT; ACFT HAD PASSED THROUGH THE OM. CAPT FINISHED COMPLETING BEFORE LNDG CHKLIST. AT THIS POINT; CIRCUIT BREAKER FOR ALT INDICATOR AND HSI POPPED. CAPT MOMENTARILY LOST INSTS UNTIL CIRCUIT BREAKER WAS RESET. ACFT BROKE OUT OF IMC APPROX 400 FT FROM THE GND AND 2 MI FROM RWY. CLRNC TO LAND WAS ISSUED BY TWR AND READ BACK BY CAPT. AFTER A NORMAL LNDG WAS MADE; ACFT EXITED THE RWY. CTLR TOLD CREW A DEV HAD OCCURRED BECAUSE OF A FAILURE TO RPT THE OM. NO ENFORCEMENT ACTION WAS TAKEN BY CTLR. CREW WAS DISTR BY POS OF ACFT ON ILS GS AND MOMENTARILY LOSS OF FLT INSTS.
