Synopsis

BIRD STRIKE. ACFT EVACUATED DUE TO A R GEAR FIRE AFTER A HIGH SPD ABORT AT 140 KTS PERFORMED BY THE PIC OF A B747-400 WHEN HEARING A LOUD BANG ON TKOF ROLL AT VTDB.

Narrative

ABORTED AT 140 KTS. V1 WAS 145 KTS. HEARD A LOUD BANG AND FELT IT. I ELECTED TO ABORT. ROLLOUT WAS ON CTRLINE AND CLRED RWY RIGHT ON TXWY S. WHEN CLRED; WE CHKED CHART AND FOUND IT TO BE IN THE MELT ZONE. I ASKED TWR IF THEY SAW ANY FIRE. THEY SAID THEY SAW SMOKE ON THE R SIDE. THE FIRE DEPT ARRIVED AND I ASKED THEM TO HAVE THE FIRE COMMANDER CHK FOR FIRE. THEY SAID THAT THE R GEAR WAS ON FIRE; SO I ELECTED TO EVAC. I ORDERED AN EVAC OUT THE L SIDE ONLY (DUE TO FIRE ON THE R SIDE). BY THE TIME WE (FO AND I) LEFT THE COCKPIT; ALMOST THE WHOLE UPPER DECK HAD BEEN EVACED; SO I ORDERED THE FO TO GO DOWN THE SLIDE AND SUPERVISE THE OPS ON THE GND. I PROCEEDED TO THE LOWER DECK AND SUPERVISED THE EVAC OF THE LOWER DECK UNTIL ALL THE PAX HAD CLRED. I THEN ORDERED THE FLT ATTENDANT TO EXIT AND I DID ONE MORE SWEEP OF THE ACFT TO CHK FOR ANY REMAINING PAX. I THEN LEFT THE ACFT. EVAC WENT EXTREMELY WELL AND THE TOTAL ACFT (365 SOULS ON BOARD) WAS DONE WITHIN 2 MINS WITH ONLY 2 INJURIES. AFTER LEAVING THE ACFT; MAINT SAID THAT THEY RECEIVED A RPT OF A SURGE ON THE #2 ENG. A PAX RPTED A 'LARGE' FLAME AND LOUD BANG OUT OF THE #2 ENG. MAINT FOUND REMNANTS OF A BIRD IN THE #2 ENG. SUPPLEMENTAL INFO FROM ACN 625960: I INSPECTED THE ENGS AND FOUND REMAINS OF A BIRD IN THE #2 ENG AND ASKED MAINT TO CONFIRM THE FINDINGS AND REMOVE A FEATHER FOR ME.
