CVG CTLR CHRONICLES NEAR SEPARATION LOSS WITH DEP ACFT AT 6000 FT.

Narrative

SIMULTANEOUS DEPS RWY 18L&R. RWY 18L SID IS TO TURN L TO 165 DEG HDG. RWY 18R IS STRAIGHT OUT. ON RWY 18R; ACR Y CAME OFF ON WRONG BEACON. ON RWY 18L; ACR X CAME OFF WITHOUT FLT PROGRESS STRIP. WE WERE HAVING PRINTER TROUBLE. I THOUGHT I GAVE RWY 18L TO ACR 80 BUT NO STRIP MARKING DUE TO NO STRIP. NO RESPONSE. ISSUED TURN TO 210 DEGS TO ACR Z TO JOIN BLUEGRASS DEP SID. ACR W WAS 30 MI SW AND I ISSUED CLB AND TURN TO JOIN SID. NO RESPONSE. ISSUED TO ACR W AGAIN AND I GOT A BAD READBACK. RESTATED INSTRUCTIONS TO ACR W. AT SOME POINT; ACR XX OFF RWY 18R ACQUIRED. ACR Y WENT THROUGH SID AND WAS CONVERGING; ALTHOUGH OUT IN FRONT OF ACR XX. ISSUED 240 DEG TURN TO ACR XX TO JOIN SID. TURNED ACR X BACK SE TOWARD SID. FACTORS: 1) BAD BEACON CODE RWY 18R DEP. 2) NO PROGRESS STRIP RWY 18L DEP AND SUBSEQUENT NO STRIP MARKING AS MEMORY JOGGER FOR ME TO VERIFY 60 OR 80 ISSUED. 3) RWY 18L ACR X DEP WENT THROUGH THE SID TOWARD RWY 18R DEP. 4) MISSED XMISSION AND READBACK OF ACR W; 30 MI SW; WAS TIME CONSUMING. 5) 6TH DAY OF MY WORK WK.
