Synopsis

AN EXPERIENCED MD10 PLT RECEIVING A LINE CHK DSNDED BELOW MCO ATC XING RESTR AFTER AN FO ADVISORY AND FAILED HIS LINE CHK.

Narrative

ORL APCH CLRED US FOR A VISUAL RWY 18R TO CROSS THE VOR AT 2500 FT. HOWEVER; I FLEW THE ILS SO WE CROSSED THE VOR AT 2000 FT. I HAD NOT NOTICED THAT HE WANTED US HIGHER THAN GS AT THAT POINT. SUPPLEMENTAL INFO FROM ACN 656616: WHEN GS CAME ALIVE; CAPT ARMED THE AUTOPLT FOR THE ILS. AS ACFT ARRIVED AT 2500 FT AND CONTINUED TO DSND (PRIOR TO THE VOR) I NOTIFIED THE CAPT OF THE RESTR. CAPT CONTINUED; LCA SAID NOTHING; CAUSING ME TO QUESTION THE CLRNC. IN THE SECONDS OF THAT THOUGHT PROCESS; WE CROSSED THE VOR ABOUT 500 FT LOW. NO MENTION BY THE TWR. LANDED SAFELY. SUPPLEMENTAL INFO FROM ACN 655975: THE CAPT CROSSED THE VOR ON THE ILS GLIDE PATH AND WAS APPROX 1900 FT AT VOR. THE FO WAS PLT MONITORING AND HE POINTED OUT THE RESTR WHEN THEY STARTED DOWN. I WAS CONDUCTING A RECURRENT LINE CHK OF THE CAPT AND WAS SITTING IN AN OBSERVER'S SEAT. I WAS ABOUT TO CORRECT THE CAPT IN THE DSCNT BUT FIGURED THAT THE FO WARNING WOULD BE COMPLIED WITH. DEBRIEF: THE CAPT WAS THINKING THAT SINCE HE WAS CLRED FOR THE APCH; THE RESTR DID NOT APPLY. I POINTED OUT THAT HE WAS IN ERROR AND HE FAILED THE LINE CHK. THIS WAS A VERY EXPERIENCED CAPT AND IT WAS VERY SURPRISING TO SEE SUCH A VIOLATION. COMPLACENCY WOULD BE A POSSIBLE FACTOR.
