B757-200 CREW RPTED CLR FOR PUSHBACK BY FLT ATTENDANTS AND GND CREW. AGENT OPENED A MAIN CABIN DOOR AS ACFT PUSHED; DAMAGING DOOR.
WE WERE IN THE BEFORE STARTING ENGS CHKLIST WHEN THE AGENT STUCK HIS HEAD IN THE COCKPIT AND ASKED IF WE WERE READY TO GO. WE REPLIED YES. WE FINISHED THE CHKLIST UP TO THE JP'S. THE #1 FLT ATTENDANT GAVE US HER CABIN READY RPT AND CLOSED THE COCKPIT DOOR. THE GND CREW CHKED IN ALL CLR BELOW; READY FOR BRAKE RELEASE. CAPT SAID STAND-BY. WE FINISHED THE BEFORE STARTING ENGS CHKLIST INCLUDING CONFIRMING ALL DOOR LIGHT EXTINGUISHED; TO PACKS OFF. I CALLED CLRNC FOR PUSH THEN SWITCHED TO GND. GND CLRED US TO PUSH. CAPT TOLD GND CREW; BRAKES RELEASED; CLRED FOR PUSH. THE PUSH CREW BEGAN THE PUSH. I HEARD A LOUD SCRAPE AND ACFT STOPPED. UNKNOWN TO US THE AGENT HAD SIGNALED THE #1 FLT ATTENDANT TO OPEN THE FORWARD ENTRY DOOR WITHOUT NOTIFYING US. THE GND CREW DID NOT SEE THE AGENT OPEN THE DOOR. WE DID NOT SEE THE DOORS LIGHTS ON THE OVERHEAD PANEL COME ON AFTER IT WAS VERIFIED OUT; NOR THE EICAS MESSAGE. DAMAGE WAS DONE TO THE FORWARD DOOR ESCAPE SLIDE ASSEMBLY. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THIS INCIDENT WAS A COMBINATION OF ERRORS. THE AGENT DID NOT ATTEMPT TO GET THE CREW'S ATTN; THE FLT ATTENDANTS DID NOT NOTIFY THE COCKPIT; THE FLT CREW DID NOT SEE THE DOOR LIGHT ILLUMINATE AFTER IT WAS OUT; AND THE PUSH CREW DID NOT SEE THE JETBRIDGE MOVE TO THE ACFT.
