Synopsis

FATIGUED E140 FLT CREW DEPLANES WITHOUT SHUTTING DOWN THE #2 ENG.

Narrative

AFTER DEPLANING ON A TERMINATION FLT IN CMH; I GOT OFF THE PLANE; BRIEFLY HELPED THE AGENT GET A FEW VALET BAGS TO THE PAX; AND WAS GOING BACK ONTO THE PLANE TO FINISH POST-FLT WHEN ONE OF THE RAMP PERSONNEL POINTED OUT THAT AN ENG WAS STILL RUNNING. I FOUND THAT THE SWITCH FOR THE #2 WAS STILL ON; AND THE REST OF THE SWITCHES WERE IN THE CORRECT POS. I ALSO REMEMBERED THAT WE HAD NOT DONE A PARKING CHKLIST AND SO I VERIFIED THAT EVERYTHING WAS ACCOMPLISHED. I THINK A NUMBER OF FACTORS CONTRIBUTED; BUT THE MAIN ONE BEING FATIGUE. I HAD BEEN UP SINCE EARLY SO THAT I COULD COMMUTE FOR AN XA20 DEP; AND IT WAS XM30 AT THE TIME OF THIS ERROR. I FELT FATIGUED. ALSO; THE GND PWR WAS HOOKED UP DURING THE PARKING FLOW; AND I REMEMBER GETTING IT TURNED ON RIGHT AWAY TO SAVE FUEL; BUT I ALLOWED IT TO INTERRUPT MY NORMAL FLOW AND NEVER CAME BACK TO THAT PART OF THE PARKING FLOW. ALSO; AT THE SAME TIME; I WAS INTERESTED IN KNOWING WHETHER MAINT WANTED THE PLANE COMPLETELY SHUT DOWN OR NOT; AND THE REST OF THE CREW QUICKLY BEGAN PACKING UP AND CALLING THE HOTEL; AS IT HAD BEEN A LONG DAY FOR ALL OF US. THERE IS PLENTY OF EMPHASIS ON CHKLIST COMPLETION. I SHOULD HAVE COMMUTED DOWN THE DAY PRIOR KNOWING THAT IT WAS GOING TO BE A LONG DAY EVEN WITHOUT A COMMUTE.
