Synopsis

B757 FLT CREW HAS AN ALTDEV HOLDING ON STAR TO LGA.

Narrative

I WAS THE PNF AND ACFT WAS GIVEN HOLDS ENRTE TO LGA. WE WERE TOLD TO SLOW TO 250 KTS JOINING THE KORRY 3 ARR FOR LGA. FINALLY ATC GAVE US A LATE HOLDING INSTRUCTION TO CROSS HOLEY AT 13000 FT; 210 KTS AND HOLD AS PUBLISHED. I REPEATED THE INSTRUCTION AND THE CAPT (PF) REPEATED; BUT I THINK HE MUST HAVE MISSED THE PART ABOUT XING THE VOR AT 13000 FT; BECAUSE ATC ASKED US TO CONFIRM OUR ALT OF 13700 FT AND I DID; THEN A FEW SECONDS LATER; ATC ASKED US TO CONFIRM WE WERE AT 13000 FT AND AT THE TIME WE WERE; SO I ADVISED ATC WE WERE AT 13000 FT. I GOT DISTR BY OTHER ITEMS IN THE COCKPIT; SO I WAS NOT FULLY MONITORING THE PF'S XING RESTR. AFTER WE DEVIATED FROM THE CLRNC; THE CAPT SAID HE NEVER HEARD ATC TELL US TO CROSS THE FIX AT 13000 FT. IT WOULD HAVE BEEN DIFFICULT TO MAKE REGARDLESS SINCE ATC WANTED US TO SLOW AND DSND AT THE SAME TIME AND I BELIEVE WE WERE ONLY 3 NM FROM THE FIX WHEN GIVEN THE CLRNC. I SHOULD HAVE TOLD ATC UNABLE. CONTRIBUTING FACTORS TO THIS DEV WERE FATIGUE. THE CAPT SAID HE ONLY GOT 5 HRS SLEEP THE NIGHT BEFORE AND I RECEIVED ABOUT 3. CREW SCHEDULING WOKE ME UP AT 3:30 AM THAT MORNING ON MY DAY OFF AND ASKED ME TO COVER A TRIP DUE TO SOMEONE SICK. THE RPT TIME WAS 6:30 AM. I COULD NOT RETURN TO SLEEP AFTER THEY CALLED AND I DID NOT WANT TO TURN DOWN THE TRIP SINCE I NEED A MINIMUM AMOUNT OF FLT TIME BY THE END OF THE MONTH TO MAKE UP FOR 50% PAY CUT. I THINK FATIGUE PLAYED A FACTOR FOR BOTH OF US AND NEXT TIME I WON'T FLY WITHOUT FEELING FULLY RESTED; SO I CAN ADEQUATELY MONITOR THE PF. FROM WHAT I SAW; THERE WERE NO CONFLICTS WITH ANY OTHER ACFT IN THE AREA AND TCAS NEVER ADVISED OF ANY CONFLICTS. WE WERE IN A HOLDING PATTERN THOUGH; BUT ENTERING IT.
