Synopsis

NEAR TRACK DEV OCCURS AFTER E145 FLT CREW -- INCORRECTLY -- MANUALLY INSERTS WAYPOINTS OF SHNON RNAV STAR TO IAD IN THE FMS ROUTE.

Narrative

WE WERE FLYING TO IAD AND WERE FILED TO FLY THE SHNON 2 RNAV ARR INTO IAD. WHILE AT THE GATE; THE FO PUT THE FLT PLAN INTO THE FMS AND I DOUBLECHKED THAT IT LOOKED RIGHT. WHEN I LOOKED OVER WHAT WAS IN THE FMS AT THE GATE EVERYTHING APPEARED TO BE RIGHT. THE SHNON 2 ARR IS SUPPOSED TO BE FLOWN HVQ-DILNN-PUTTZ-FINKS-DOCCS-LDN-SHNON-ROYIL-KEWPY-ELSIN. WHEN WE CROSSED HVQ; I NOTICED THAT THE PLANE WAS HEADING TO SHNON INSTEAD OF DILNN. AS SOON AS I NOTICED (PRETTY MUCH RIGHT OVER TOP OF HVQ); I HAD THE FO ASK ATC WHAT THEY SHOWED US DOING AFTER HVQ. ATC REPLIED THAT THEY SHOWED US FILED FOR THE SHNON 2 ARR AFTER HVQ. WE ADVISED THEM THAT SOMEHOW WE HAD HVQ-SHNON AND THAT WE WOULD FLY THE ARR. AT THAT POINT ATC GAVE US A CLRNC OF DIRECT DILNN THEN JOIN THE SHNON 2. WE CONTINUED THE FLT AND LANDED AT IAD WITHOUT FURTHER INCIDENT. I BELIEVE THE EVENT HAPPENED BECAUSE I MIGHT HAVE SEEN WHAT I WANTED TO SEE IN THE FMS WHILE WE WERE AT THE GATE. CONTRIBUTING FACTORS WERE THE FACT THAT IT SEEMS LIKE THE SHNON 2 ARR IS PRE-LOADED INTO THE FMS WRONG. WHEN MY FO TRIED TO PUT THE SHNON 2 ARR INTO THE FMS WHEN WE WERE OVER THE TOP OF HVQ (USING THE NAV-ARR-STAR FEATURE OF THE FMS); IT STILL WENT STRAIGHT FROM HVQ TO SHNON INSTEAD OF HVQ TO DILNN. SO I HAD THE FO PUT THE FIXES IN THE FMS MANUALLY. OTHER CONTRIBUTING FACTORS INCLUDE: 2 MEL'S THAT WERE ON THE PLANE WHICH WAS THE PACK 1 AND THE AUTOPLT WERE DEFERRED. THE COCKPIT STAYED AT ABOUT 28 DEGS C MOST OF THE DAY AND THE COMBINATION OF BEING HOT WHILE HAND FLYING IN BAD WX AND AN EARLY RPT TIME FOR WORK ENDED UP TAKING A TOLL ON MY MIND. ALSO WHILE WE WERE FUELING THE R WING TOOK MORE FUEL THAN THE L WING DID FOR A WHILE WHICH SET OFF THE FUEL IMBAL EICAS MESSAGE. THE FUEL SYS THEN CORRECTED ITSELF BEFORE THE FUELER WAS DONE FUELING. WITH THOSE DISTRS AND THE DELAYS; IT SEEMS LIKE I WAS ABLE TO OVERLOOK A MISTAKE IN THE FMS ROUTING.
