Narrative

INADVERTENT OVERFLY OF THE INITIAL EDDY CURRENT INSPECTION REQUIRED BY HARTZELL AIRWORTHINESS DIRECTIVE NOTE 2008-13-28. THE EFFECTIVE DATE OF THE AIRWORTHINESS DIRECTIVE NOTE JUST MISSED BEING DISSEMINATED TO US AT A SCHEDULED INSPECTION AND THEN AN INDIVIDUAL TECHNICIAN INCORRECTLY UTILIZED THE SEARCH FUNCTION ON THE AIRWORTHINESS DIRECTIVE SOFTWARE MORE FREQUENT REVIEW OF AIRWORTHINESS DIRECTIVE COMPLIANCE RECORDS AND ENHANCED TRAINING FOR TECHNICIANS USING THE SEARCH SOFTWARE WILL PREVENT ANY FURTHER OCCURRENCE. REPORTER STATED HE HAD TO GET A FERRY PERMIT IN ORDER TO GET THE PIPER SEMINOLE PA44-180T TO A LOCATION TO HAVE THE EDDY CURRENT INSPECTION ACCOMPLISHED. REPORTER STATED A PREVIOUS AIRWORTHINESS DIRECTIVE (AD) WAS ISSUED FOR SIMILAR RIGHT-HAND ROTATING HARTZELL PROPS. THE AD REFERS TO CRACKS EMANATING FROM THE LUBRICATION HOLES IN THE PROP HUB; BASICALLY FROM STRESS LOADS. THESE HOLES ARE MACHINED TO HOLD THE ZERK TYPE GREASE FITTINGS. THE NEWER; IMPROVED HUBS HAVE MORE METAL AROUND THE MACHINED ZERK FITTING HOLES.
