Synopsis

ACR WDB ENGINE FIRE WARNING JUST AFTER TKOF. RETURN LAND.

Narrative

VERY SOON AFTER TKOF ON RWY 08 THE R ENG FIRE WARNING SOUNDED. WE DECLARED AN EMER; ADVISED THE TWR WE WERE RETURNING TO LAND ON RWY 08. BEGAN A CLIMBING L TURN AND BEGAN EXECUTING THE ENG FIRE WARNING CHKLIST PROCEDURE. WE SET UP A VFR DOWNWIND AT 1500 FT. IN EXECUTING THE EMER PROC IT BECAME APPARENT THAT THE R THROTTLE WAS STUCK IN THE TKOF THRUST POS. THE RIGHT ENG HAS SECURED WITH THE FUEL LEVER AND THE FIRE HANDLE. THE FIRE LIGHT NEVER WENT OUT. THE #1 FLT ATTENDANT WAS ADVISED THAT WE HAD A FIRE WARNING; WE WERE RETURNING TO SJU; ASAP AND EVAC WAS A DISTINCT POSSIBILITY. THE LNDG WAS NORMAL. SJU TWR ADVISED US THERE WAS NO VISIBLE FIRE. WE CLRED THE RWY; PUSHED THE BRAKES AND EVACUATED THE ACFT. 6 MINS ELAPSED BTWN FIRE WARNING AND EVAC. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING: THAT THE ACFT HAD ARR AT SJU WITH HI PRESSURE BLEED PROB NOTED IN THE ACFT LOG BOOK. MAINT REPLACED THE BLEED VALVE AND A DUCT CLAMP EITHER CAME LOOSE AFTER TKOF OR WAS NEVER INSTALLED. ENG OVER TEMP OCCURRED SOMETIME DURING THE INCIDENT CAUSING SEVERE ENG DAMAGE. ENG WAS REMOVED AND REPLACED. RPTR SAID HE HAS HEARD OF ANOTHER INCIDENT SIMILAR TO THIS AND IT IS RUMORED AS BEING A BLEED PROB. SPECULATION IS THAT OTHER ACFT USING THIS SAME ENG ARE HAVING HI STAGE BLEED PROBS.
