THE NTSB RECOMMENDS THAT THE FEDERAL AVIATION ADMINISTRATION: REGULARLY MONITOR THE QUALITY ASSURANCE AND TRAINING PROGRAM AT THE CHICAGO O'HARE AIR TRAFFIC CONTROL FACILITY TO REQUIRE THAT AIR TRAFFIC CONTROLLERS WHO HAVE BEEN INVOLVED IN AN OPERATIONAL ERROR ARE COUNSELED, TRAINED, AND RECERTIFIED AS REQUIRED BY THE FACILITY OPERATION AND ADMINISTRATION HANDBOOK 7210.3H AND THAT APPROPRIATE ENTRIES TO REFLECT THESE ACTIONS ARE MADE IN THE CONTROLLER'S TRAINING RECORDS.
