Event Description

During a flight, the pilot obtained fuel samples through the remotely operated gascolator but detected no water. The aircraft's wing and fuselage tanks lacked quick drains, preventing direct sampling. During takeoff, a power loss occurred at an altitude between 50 and 100 feet above ground level. The pilot switched the fuel tank selection from left to right main and engaged the boost pump. Engine power returned at the moment the aircraft struck utility lines near the departure end of the runway. The aircraft then impacted a fence before coming to rest.

Fuel System Findings

After the accident, direct access to the left main tank revealed the presence of water, which was pink in color. Pink stains were also observed on the tank wall above the aft fuel outlet port. The outlet ports of the interconnected main tank cells were located 3/8 to 1/2 inch above the tank bottoms.

Previous Occurrence

Approximately three weeks earlier, a similar power loss had occurred. Subsequent maintenance found a loose left tank fuel cap that could have allowed water ingress. The cap was adjusted for a tighter fit. The aircraft was flown twice after that adjustment with no evidence of water in the fuel or other discrepancies.
