What happened

On November 18, 2011, an Embraer EMB-145LR, operating as American Eagle flight 4125 (registration N620AE), landed on runway 24L at Toronto Lester B. Pearson International Airport (CYYZ). After exiting the runway at taxiway D4, the crew was instructed to hold short of runway 24R. However, the aircraft proceeded to cross the hold line for runway 24R before coming to a complete stop.

During this time, Air Canada flight 614, an Airbus A319, was cleared for takeoff on runway 24R. The A319 became airborne approximately 3,800 feet before reaching taxiway D4, passing over the stopped Embraer at an altitude of approximately 600 feet. There were no injuries and no damage reported during the event.
