Notification and International Cooperation
On June 6, 2012, a Sikorsky S58 helicopter was involved in an accident in Peru. The Comisión de Investigación de Accidentes de Aviación (CIAA) of Peru, the national aviation safety investigation authority, notified the United States National Transportation Safety Board (NTSB) of the occurrence.
Accredited Representative Appointment
In accordance with the provisions of ICAO Annex 13, the NTSB appointed a U.S. Accredited Representative to participate in the investigation. The NTSB serves as the State of Manufacturer and Design for the Sikorsky S58 helicopter.
Investigation Lead
The CIAA will lead the investigation and release all related investigative information. No further details have been disclosed at this time.
