What happened

On June 12, 2012, at approximately 2140 UTC, a Cessna 150M, registration HI-561, was conducting a routine instructional flight departing from Santiago International Airport (MDST) in Santiago de los Caballeros, Dominican Republic. During the flight, the tip of one propeller blade separated from the propeller.

Following the separation, the pilot performed a forced landing in Licey, Dominican Republic. The aircraft sustained substantial damage to the airframe. The two pilots on board sustained 2 minor injuries, and there were 0 fatalities.
