What happened
On July 12, 2015, a Bombardier/DHC-8-402Q operated by PLL LOT SA was involved in a ground incident at Warsaw Chopin Airport (EPWR). After the aircraft had landed and taxied to its parking position, a ground handling employee was in the process of placing chocks under the wheels. During this procedure, the worker struck their head against the anti-icing probe tube.
The impact resulted in a head wound that required medical treatment and stitches at a hospital. The collision also caused slight bending of the anti-icing probe tube, which was documented via photography and classified as a technical defect.
The investigation
The investigation focused on the physical circumstances of the ground handling task and the condition of the aircraft following the contact. Following the incident, the aircraft's technical status was reviewed in consultation with the Maintenance Control Center (MCC). Based on the findings and in accordance with MEL 30-80-2, the aircraft was cleared to continue flight operations with passengers.
Findings
- The primary cause of the incident was inattention by the ground handling employee during the chocking process.
- The incident is classified as an occupational accident occurring during routine ground operations.
