What happened
On June 4, 2016, at approximately 1500 UTC, a Wag-Aero Super Tri-Pacer, registration CGSMJ, struck terrain in the vicinity of Williston Lake, British Columbia, Canada. The accident occurred under circumstances that were not immediately known. The flight involved a pilot and a passenger, both of whom sustained 1 fatal injury and 1 fatal injury, totaling 2 fatalities.
The Transportation Safety Board of Canada led the investigation into the event.
