Accident Overview
On August 27, 2016, at approximately 1025 coordinated universal time, a Cessna 182Q aircraft, South African registration ZS-PHB, was destroyed when it impacted terrain shortly after takeoff from the Komati Gorge Lodge private airstrip in Mpumalanga Province, South Africa. The pilot, who was the sole occupant, sustained fatal injuries. The flight was conducted as a local personal flight under visual meteorological conditions, operating in accordance with the civil regulations of South Africa.
Investigation
The South African Civil Aviation Authority (SACAA) Accident and Incident Investigations Division (AIID) is leading the investigation into the accident. As the State of Manufacture and Design of the engine, the United States National Transportation Safety Board (NTSB) has appointed a US Accredited Representative to assist in the inquiry, following the provisions of ICAO Annex 13.
