Accident Overview
On December 7, 2016, at approximately 1150 UTC, an Industria Aeronautica Neiva S/A EMB 721D, Brazilian registration PT-REI, was destroyed after colliding with trees and terrain during a forced landing. The aircraft was registered to and operated by a foreign individual for a non-commercial, non-U.S. flight. Visual meteorological conditions prevailed at the time; flight plan information is unknown. The flight originated one minute earlier from Manaus/Flores Airport, Manaus, Brazil, with a destination of Novo Aripuana Airport, Aripuana, Brazil.
Sequence of Events
Shortly after takeoff from runway 11, witnesses reported that the engine experienced a loss of power and the airplane was observed losing altitude. During a forced landing in a wooded area east of the airport, the airplane impacted trees and came to rest inverted on the ground. The commercial pilot and five passengers sustained fatal injuries. One passenger survived the impact but died while hospitalized shortly after the accident.
