What happened

On November 26, 2018, a Boeing 767-300 operated by Ethiopian Airlines experienced a failure of the No. 2 (right) engine fan blades during its takeoff from Addis Ababa Bole International Airport (ADD) in Ethiopia. Following the failure, the aircraft, registration ET-AMG, returned to the airport. The landing was completed without incident, and there were no injuries reported to the occupants.

The investigation

Post-flight examination of the aircraft identified that two fan blades had fractured at mid-span. The inspection also revealed damage to the inlet and the fan cowl, as well as minor impact damage to the fuselage.
