What happened
On 16th May 2024, a Bell 505 helicopter, registered as SP-RMK, was involved in an accident in Duchnów, located near Mińsk Mazowieck. The investigation into the occurrence, identified as accident no. 2024-0025, is currently being conducted by the State Commission on Aircraft Accidents Investigation (SCAAI).
The investigation
The SCAAI Investigation Team has performed extensive technical and operational evaluations to determine the circumstances of the event. The scope of the investigation has included an examination of the helicopter's airframe and engine, as well as a review of the technical and operating documentation. Furthermore, the team has evaluated crew performance, Air Traffic Services activities, and the effectiveness of rescue, firefighting, and post-landing evacuation procedures.
To assist in the technical analysis, the SCAAI requested assistance from the French Bureau d’Enquêtes et d’Analyses (BEA) to extract and analyze data from the aircraft's Full Authority Digital Engine Control (FADEC) system. The investigation is being conducted in cooperation with the ICAO-designated Accredited Representative and technical advisers from the BEA.
Findings
At this interim stage, the investigation team has concluded its primary field activities and is currently drafting the Final Report. While the investigation is ongoing, the commission has not yet released a definitive determination of the causes of the accident.
