What happened
On the morning of 9 September 2025, a CH-601 Zodiak with registration OK-LUA60 took off from the Lipowa airfield in Poland. The flight, operated by two men, was intended to travel to an airport in the Czech Republic. At the time of departure, the Żywiec Basin was covered in dense fog, which significantly restricted both horizontal and vertical visibility.
CCTV footage from the airfield confirmed that the aircraft underwent engine testing and taxied for takeoff at approximately 9:27 AM. The aircraft lifted off at 9:32 AM, initially climbing on a clear path. However, shortly after takeoff, the aircraft disappeared into the fog. Witnesses reported seeing the aircraft attempt a go-around over the airfield before it disappeared from view. Approximately three minutes after takeoff, the aircraft struck a nearby field at a high angle and immediately burst into flames. The impact caused the fuel tanks to rupture, fueling a rapid fire. Both occupants, aged 70 and 50, sustained fatal injuries and could not be rescued.
The investigation
The PKBWL investigation utilized CCTV footage, witness statements, and a physical examination of the wreckage. The investigation established that the aircraft's engine was likely shut off by the pilots just before impact, as the propeller blades showed no signs of mechanical damage or windmilling. The investigation also noted that the aircraft lacked the necessary instrumentation to permit flight in conditions of no external visibility (IMC). Furthermore, the aircraft was likely at or exceeding its maximum permissible takeoff weight due to the combined weight of the crew and fuel.
Findings
- The primary cause of the accident was the decision to take off in weather conditions that did not meet VFR minimum requirements.
- The crew lost spatial orientation due to the dense fog, making it impossible to maintain control or find the airfield.
- High-intensity icing was likely present in the carburettor air intake system, which may have reduced engine power and prevented the aircraft from climbing above the fog layer.
- The aircraft was not equipped with instruments capable of supporting flight in deteriorating weather.
