Occurrence Details
On April 14, 2025, at approximately 1:04 PM local time (1704 UTC), an Embraer EMB-145LR aircraft with Bahamian registration C6-SAW, operated by WesternAir as flight WST402, was involved in a serious incident shortly after departing from Lynden Pindling International Airport (MYNN), Nassau, Bahamas. The aircraft was destined for New Bight International Airport (MYCB), New Bight, Cat Island, Bahamas. There were 3 crew members and 47 passengers on board.
According to the pilot in command, shortly after takeoff from MYNN while passing through 5,000 feet, the flight crew heard a loud audible bang emanating from the right engine, which shook the aircraft. The flight crew observed a significant increase in the corresponding Turbine Inlet Temperature (TIT). In accordance with emergency procedures prescribed by the aircraft manufacturer, the crew shut down and secured the right engine. Air traffic control at MYNN was notified of the emergency, and requisite emergency personnel were informed. The flight crew returned to MYNN, where a safe landing was executed without further incident. There were no reported damages to the aircraft and no injuries were reported.
Aircraft and Engine Information
The aircraft, an Embraer EMB-145LR, was manufactured in 2001 with serial number 145435. It was powered by two Rolls-Royce AE 3007A1 turbofan engines. The right engine, serial number CAE311249, had a Time Since New (TSN) of 42,579 hours and 38,521 cycles since new. Its last shop visit occurred 699.3 hours prior to the occurrence on April 14, 2025.
Maintenance Findings
The AAIA received documentation of aircraft maintenance records taken on April 16, 2025. These records noted that the #2 engine (right) had normal visual inspections, the fan rotor spanned freely, but the N2 train/shaft was seized. Review of records revealed compliance with the manufacturer's maintenance manual and inspection report. The inoperative engine was replaced with a newly overhauled engine, serial number CAE311982, in accordance with the applicable manuals.
