What happened
On January 16, 2009, at approximately 11:30 local time, a Bell 206 B helicopter, registration EC-FHX, was conducting agricultural spraying operations in Cuevas de Almanzora, Almería. The aircraft, operated by Rotorsun, S. L., was performing its second flight of the day.
As the pilot took off, reaching an altitude of approximately 2 meters and a distance of 30 meters from the loading point, the downwash from the main rotor lifted a piece of plastic sheeting used to cover crops. This plastic material became entangled around the main rotor mast, obstructing the air intake. This obstruction led to an immediate engine failure, causing the helicopter to descend rapidly and perform a hard landing.
The investigation
The investigation focused on the sequence of events leading to the engine shutdown and the operational procedures of the operator. The crew examined the circumstances surrounding the takeoff and the presence of loose materials in the flight area. The investigation also reviewed the company's operations manual, which outlines procedures for agroforestry work in both large and small areas, including the potential use of ground support personnel, though it lacked specific details regarding the exact tasks required of such personnel.
Findings
- The pilot was highly experienced in these types of agricultural operations.
- The specific plastic sheet involved appeared to be folded and secured with weights, similar to other covers in the area, making its sudden movement difficult to predict.
- The primary cause of the accident was the entanglement of plastic sheeting around the rotor mast, which blocked the engine's air intake.
- The aircraft sustained significant damage to the main rotor and landing gear, and the tail rotor drive shaft was broken due to a blade strike against the tail cone.
- The pilot remained uninjured and was able to exit the aircraft independently.
