What happened
On 7 June 2021, a Bell 214ST helicopter, operated by Helicopter Transport Services (Canada) Inc., was performing fire suppression duties near Nipigon, Ontario. While returning to the aerodrome with a collapsible bucket attached to a long line, the pilot experienced a vibration in the tail rotor pedals along with a grinding sound.
Shortly after these symptoms appeared, low oil pressure warning lights for both tail rotor gearboxes illuminated. This was immediately followed by a total loss of yaw control. In response to the emergency, the pilot released the long line and declared a Mayday. The aircraft performed a forced landing, striking the terrain heavily and coming to rest on its left side.
The investigation
The investigation focused on the mechanical failure of the tail rotor system and the subsequent loss of control. The pilot, who was the sole occupant of the aircraft, was wearing a helmet and a lap belt at the time of the impact. Following the heavy landing, the pilot was able to exit the aircraft after shutting down the engines. The emergency locator transmitter was activated during the event, and no fire was reported at the scene.
Findings
