What happened
On 13 September 2025, a float-equipped De Havilland DHC-2 MK.1, operated by MHA Enterprises Ltd., was performing a visual flight rules flight in Manitoba. The aircraft was traveling from a float base located at St. Theresa Point toward Makepeace Lake, carrying one pilot and four passengers.
As the aircraft approached its destination, approximately half a nautical mile from Makepeace Lake, it struck the terrain. The impact was followed by the ignition of a post-crash fire. Following the accident, both the pilot's mobile phone and the aircraft's emergency locator transmitter sent signals that alerted emergency responders. Personnel from the Island Lake RCMP and search and rescue technicians subsequently arrived at the crash site.
The investigation
The TSB investigation focused on the circumstances surrounding the impact and the subsequent fire. The investigation established that the aircraft was operating under visual flight rules at the time of the accident and confirmed the presence of five occupants on board.
