What happened
On November 18, 2024, a 369E helicopter, registration SE-JVM, was conducting a specialized power line inspection flight at Enköping/Långtora Airport in Uppsala County. Following a refueling stop, the crew—consisting of a pilot and a system operator—attempted a takeoff from a grassy area.
As the aircraft entered a hover just above the ground, it began to rotate clockwise. The pilot attempted to correct the movement using full pedal travel, but the rotation continued, increasing in speed. The helicopter rose to approximately 15 meters before descending and striking the ground. The aircraft completed six full rotations before the impact. Both crew members sustained minor injuries, aided by the use of helmets and four-point seat belts.
The investigation
The Swedish Accident Investigation Authority (SHK) examined the wreckage, interviewed the crew and witnesses, and analyzed technical data from the aircraft's measurement system. Investigators also performed specialized examinations of the engine, fuel, and oil, as well as a fractographic analysis of various components.
Technical inspections of the engine and the rotor system revealed no mechanical malfunctions or pre-existing faults that could have caused the loss of control. The investigation determined that the damage to the drive shaft and yaw control components occurred during the impact with the ground.
