What happened
A cargo flight traveling from Tbilisi to Omsk was conducting its arrival procedures at Omsk-Severny and encountered significant weather-related visibility issues. The four-engine aircraft was transporting a payload of 10,400 kilograms of various goods, along with a total of nine people, consisting of six crew members and three passengers.
During the short final phase of the approach, the flight crew proceeded below the minimum descent altitude without achieving visual contact with the runway or approach lighting systems. As a result, the aircraft collided with trees in a forested area situated approximately 2,130 meters before reaching the runway threshold. The impact was followed by a post-crash fire that completely destroyed the airframe. There were nine fatalities and no survivors.
Findings
Investigations into the accident determined that the primary factor was the crew's decision to descend below the MDA despite the lack of visual references. At the time of the incident, weather conditions had reduced visibility to levels below established minimums. Furthermore, the crew was unable to execute a go-around procedure or divert to the designated alternate airport in Tyumen because the aircraft lacked sufficient fuel reserves to complete such an maneuver.
