Aircraft crash during approach to Kharkiv-Osnova Airport

3 fatalities • Kharkiv-Osnova, Ukraine • Landing (descent or approach)

A positioning flight operating from Moscow to Kharkiv resulted in a fatal accident when asymmetric flap deployment caused an uncontrolled descent.

What happened

A positioning flight, intended to collect passengers for a charter service to Lyon, was conducting its final approach to runway 07 at Kharkiv-Osnova Airport. While the aircraft was approximately 4,600 metres from the runway threshold, the crew initiated the deployment of the flaps. During this maneuver, a significant mechanical discrepancy occurred: the right flap extended to 20 degrees as intended, but the left flap only reached an angle of 6 degrees.

This flap asymmetry triggered a sudden roll to the left, leading the aircraft into an uncontrolled descent. The aircraft struck the shore of Lake Komsomolsk, approximately 4,300 metres from the runway threshold and 750 metres to the left of the extended centerline. The impact caused the aircraft to disintegrate, with debris scattered 140 metres from the primary impact site. The main wreckage subsequently sank into the lake. There were three fatalities among the crew members.

Findings

The investigation identified that the asymmetric deployment of the flaps was the primary factor leading to the loss of control during the approach phase.

Probable cause

The aircraft entered an uncontrolled descent due to an imbalance in flap extension between the left and right wings during final approach.
