Fatal Accident Involving Piper PA-18 at Faßberg Airfield

Casualties unknown • Militärflugplatz Faßberg, DE

A pilot was killed following a loss of control during an approach at Faßberg Airfield after reporting elevator issues.

What happened

On January 2, 2025, a Piper PA-18 took off from runway 27 at Faßberg Airfield at 10:54 AM for a local flight intended to maintain the pilot's license. Shortly after departure, the pilot contacted Langen Information to state they intended to remain in the vicinity of the airfield. At 11:00 AM, the pilot reported experiencing problems with the elevator. In response to the pilot's request for emergency services, air traffic services coordinated with the airfield fire department.

During the subsequent approach, the pilot became unresponsive to radio calls. Radar data showed the aircraft's last position at the eastern boundary of the airfield at 11:01 AM. A witness observed the aircraft on final approach performing a steep climb before entering a steep descent with the left wing dropping. The aircraft struck the ground in a grassy area near the threshold of runway 27. The pilot sustained fatal injuries and passed away in the hospital later that evening.

The investigation

The BFU examined the wreckage of the Piper PA-18, the flight path via secondary radar data, and the aircraft's maintenance history. The investigation included an analysis of the cockpit trim settings and the physical condition of the control components. The aircraft, a 1954 model, was maintained by a CAMO and had undergone a 100-hour inspection in June 2024.

Findings

  • The aircraft sustained heavy structural damage, including a compressed fuselage, a broken engine mount, and deformed wings.
  • The cockpit trim was set to a nose-heavy (down) position.
  • The trim spindle was found to be slightly bent, which caused it to require increased physical force to move the trim out of the upper adjustment range.
  • While the control cables and surfaces remained connected and functional, the mechanical resistance in the trim mechanism was a significant factor.

Probable cause

The accident was caused by a loss of control during the landing approach, likely exacerbated by a bent trim spindle that required excessive force to adjust, preventing effective control of the aircraft's pitch.
