What happened
While performing a takeoff roll at Loring AFB, the flight crew identified mechanical problems involving the water injection system. In response to these issues, the captain initiated an aborted takeoff. However, the aircraft was unable to decelerate sufficiently within the available runway length.
As a result of the overrun, the aircraft type (not specified) exited the paved surface and descended into an embankment. The impact caused the airframe to break into two separate sections, and a fire subsequently broke out at the wreckage site. Despite the intensity of the flames, all seven fatalities were avoided as every occupant was successfully rescued from the scene. The aircraft was completely destroyed during the incident.
Findings
Investigations into the accident determined that the primary reason for the aborted takeoff attempt was a malfunction within the water injection system. This failure was specifically attributed to frozen water within the system components.
